**FOUR STEPS TO REGISTER FOR FALL SPORTS**
|STEP 1||Get a physical!|
Parent or Guardian fill out the Section 6 Health History form. Take the Section 6 Health History form along with the Section 7 physical form to the Authorized Medical Examiner (AME) of your choice. The Section 7 physical form must be completed and signed by the Authorized Medical Examiner (AME) performing the student’s physical evaluation. Please note the following:
|STEP 2||Register online at FamilyID.com (now called ArbiterSports)|
If this is the first time registering with FamilyID now called ArbiterSports, click "CREATE ACCOUNT" in the upper right corner of the website. Follow directions to create your account. If you need assistance, please contact Arbiter Sports support at 1-800-311-4060.
Once you have created your account, log in and search for Butler Area School District.
Please note: Pay close attention to the electronic signatures while completing the registration. Six of the agreements require BOTH the student's electronic signature as well as the parent's/guardian's electronic signature.
|STEP 3||Turn in the Section 7 Physical Form and Athletic Fee|
Turn in your child's Section 7 Physical Form and the athletic fee to the Athletic Office (located in the Senior High School gymnasium) by the registration due date show in the table at the top of the page.
If you wish to drop off your child's registration, there is a drop-box located inside the front doors of the Senior High School gymnasium on the table by the ticket booth.
If you wish to mail in your child's registration paperwork, please mail it to: Butler Senior High School Athletic Office, 120 Campus Lane, Butler, PA 16001.
If you want to submit your child's registration paperwork electronically, it may be uploaded into your child's FamilyID registration.
The Athletic Fee is a one-time per school year fee of $85.00. The Butler Area School District will only accept cash or a check at this time (made payable to BASD or Butler Area School District).
Athletic fee exceptions are as follows:
1. If you submitted the National School Lunch application through the Food Service Department for your child to receive free lunches during the 2022-2023 school year and were approved, there is no athletic fee due.
2. If you submitted the National School Lunch application through the Food Service Department for your child to receive reduced priced lunches during the 2022-2023 school year and were approved, the athletic fee is $42.50.
|STEP 4||Get your ImPACT TEST!|
All student-athletes that participate in a contact sport must have a valid ImPACT test on file with the Athletic Trainer's office prior to the first official practice or tryout. ImPACT Tests are valid for a period of two years. NO ATHLETE PARTICIPATING IN A CONTACT SPORT WILL BE PERMITTED TO TRYOUT OR PRACTICE UNLESS THE STUDENT-ATHLETE HAS A VALID IMPACT TEST ON FILE. ImPACT Testing Dates are as follows:
